Out of Town Measurement Form

Please visit your local Formal Wear Retailer to be measured for your tuxedo. Some retailers may charge a small fee for this service. Then, fill out this form and press submit. Fields marked with an asterisk (*) are required in order to process. When you arrive in Gainesville the week of the wedding, come by Jay’s to pick up your tux. You should try on your entire ensemble to make sure it properly fits before leaving the store. We will be able to handle any last minute alterations at that time.

Please, do NOT to measure yourself, have a relative/friend measure you or guess at your measurements! Current measurements are the most accurate! Even though you may have been measured for a tuxedo recently, measurements often change. To play it safe, it is always best to be measured again.
